DutiesThe employee is responsible for discharge planning which involves coordinating activities for seriously mentally ill clients in transition from the hospital to the community, i.e., housing, medications, financial entitlements, follow‑up activities. The position is in the Behavioral Healthcare Division's Client Services Entry unit and functions as the liaison to Virginia's local state Psychiatric Hospital, i.e., the Northern Virginia Mental Health Institute.Participating in weekly discharge planning activities.Providing case coordination of clients in the hospital.Making linkages for needed services.Arranging transportation of clients for placement outside the area.Maintaining case records contacts.Attending treatment team meetings.Communicating with a variety of mental health and social service professionals. The employee works independently and as part of interdisciplinary teams at the mental health center and hospitals.QualificationsMinimum: Full Performance: Master's degree in a clinical discipline such as Counseling, Psychiatric Social Work, Psychology, Art Therapy, Psychiatric Nursing, or other directly related clinical discipline that includes a clinical practicum involving diagnostic and treatment services, plus two years post‑Master's experience providing clinical therapy and other mental health services, including the application of diagnostic and treatment services.Substitution: Directly‑related higher level clinical degrees may substitute for the Master's degree requirement and one year of experience. Work experience must have been gained after acquisition of the higher degree.Desirable: Experience with seriously mentally ill adults; experience with community‑based mental health programs; and/or experience with discharge planning in a hospital setting.Work Hours & Employment ParametersWork hours: 20 hours per week between Monday and Wednesday 8 am‑4:30 pm; Thursday 8:00 am‑12:30 pm.This position may be filled at either full performance or trainee level. A trainee may receive up to two years of training; upon completion the employee must meet experience, training and proficiency requirements for the journey level. At that point a one‑year probationary period commences.Trainee salary range: $20.13 – $33.92 per hour.BenefitsBenefits packages vary by appointment type: permanent full‑time, permanent part‑time, limited term, temporary regular, and temporary seasonal/occasional.
