    SORTS Community Reintegration Specialist

    The Sex Offender Rehabilitation and Treatment Services (SORTS) program is seeking a licensed social worker to become the new Community Reintegration Specialist. This is a unique and exciting role, and will involve a variety of tasks. This individual will work closely with clients who are the highest level in the program work toward conditional release in groups and case management. They will also assist clients who have a conditional release transition into the community.

    This team member will be an integral part of the treatment team on the Pre-transition ward and the SORTS Program. They will work closely with their interdisciplinary team and also members of the Community Review Committee. A key responsibility is assisting clients who are candidates for and receive a conditional release from the court. For clients going back into the community, this includes helping with community passes and employment matters. When a conditional release is granted for medical reasons, the Community Reintegration Specialist may aid in securing a placement in a nursing home for that client.

    Salary is up to $67,000 per year depending on experience.
