(Job #542554) Student Services Professional III, CARE Coordinator, Salary Range: $5,276 - $7,517 monthly. Hiring Range: $5,276 - $5,504 monthly. This is a full-time, benefited, exempt, 12-month pay plan permanent position with a one-year probationary period with the Dean of Students. Position Summary: Under the direct supervision of the Dean of Students, the incumbent is responsible for a variety of work assignments and duties which support currently enrolled Cal Poly Humboldt students as well as formerly enrolled students who wish to re-enroll.
The incumbent assists students in navigating and accessing campus and community resources; this includes referrals to both on and off campus community providers, exploration of and referral for behavioral and medical health concerns, coordination and follow-up during and after hospitalization and/or medical leave of absence, health and safety referrals, problem resolution, managing multiple or complex medical and/or mental health needs, crisis intervention/management, and conflict resolution.
The incumbent works closely with university staff and faculty across campus and particularly with the Office of the Registrar, Educational Opportunity Program (EOP)/Student Support Services (SSS), Learning Resources Network departments, Housing & Residential life, Financial Aid, Counseling and Psychological Services, the Health Center, off-campus agencies (e.g. health clinics, hospitals including County Mental Health, mental health therapists, city and county social services), and families/ friends/guardians of students in order to support the student success of at-risk university students referred by university personnel. Presenting student issues include addressing problems of considerable difficulty and where guidelines and precedents do not exist or are not applicable. The incumbent also is involved in student threat assessment and the development of appropriate response. Additionally, the incumbent will be responsible for building and maintaining collaborative working relationships with hospital social workers to ensure an informed approach to student's post-hospitalization follow up. It is essential that the incumbent is up to date and stays abreast of research and best practices in crisis intervention and resource management. Evening and weekend hours are occasionally required.
Key Responsibilities: Caseload Management - 30% - Utilizes basic intervention and engagement techniques to assess the level of student need for personal, medical, mental health, social interpersonal assistance, and other services.
- Provides referrals to students and assists them in developing a plan for accessing support services appropriate to their needs and provides appropriate follow-up and advocacy.
- Advises the Dean of Students of Student Rights and Responsibilities regarding appropriate intervention strategies for students of concern. This position may act as Lead Care Coordinator.
Case Management Follow Up: - Maintains communication with students, faculty and staff to ensure resolution to issues/crisis that have negatively affected the student.
- Maintains complete and accurate records on students served, specific services provided, and referrals.
Intervention - 30% - Interpersonal & Developmental Assessment & Referral:
- Assesses student progress in their general development towards student autonomy and leadership. Provides coaching and referral in basic communication skills, resolving interpersonal conflicts, developing positive goals and strategies for personal success, exploring alternatives when dealing with family and personal problems, loneliness, cultural isolation and transitional academic adjustment.
- Provides referrals and assists students in accessing appropriate Student Health Services, Counseling and Psychological Services, local county services for crisis intervention and psychological services as well as referral to appropriate Academic and Learning support services.
- Other Assessment and Referral:
- Provides housing assistance, information on cultural and social activities, health and legal referrals, and referrals to other campus and community departments and agencies when appropriate.
- Assists students in planning and transitioning through each semester and academic year, as the need arises.
- Triage crisis intervention response with Dean of Students of Student Rights & Responsibilities. Also serves as member of the campus student behavior intervention team.
- Serve on the Student Behavioral Intervention Team to assist with assessment of student needs and crisis.
Advocacy - 25% - Communicates along with/on behalf of students of concern with various on/off campus resources. Communicates with campus departments, faculty and programs re: personal and community-wide crisis affecting the student and facilitates medical withdrawals and educational leaves of absence as appropriate.
- Advocates for students and student groups of concern/ at-risk students. Identifies institutional barriers and designs appropriate interventions and solutions within established precedents and guidelines and where precedents and guidelines do not exist, addressing complex student issues.
Coordination on/off campus offices and other duties - 10% - On-going consultation with other campus offices and personnel regarding individual and general needs of students of concern/at-risk students enrolled at Cal Poly Humboldt.
- Participates in university wide programming, including social, cultural, and educational activities, as appropriate to the assigned work in serving students of concern/ at-risk student population.
- Provides training to on-campus community concerning appropriate responses in working with students of concern.
- Represent Cal Poly Humboldt as a member of the Higher Education Case Managers Association and other professional associations.
- Design programs and publications and conduct trainings to educate the campus community about student wellbeing and identifying and responding to students in distress.

Job Specific Knowledge, Skills, and Abilities - Knowledge of crisis intervention, advocacy, and case management.
- General knowledge of university academic and student services and other off-campus resources, especially those frequently utilized by the target population.
- Working knowledge of Trauma Informed Care Model.
Minimum Qualifications: Experience: Possession of the below knowledge and abilities is typically demonstrated through the equivalent of three years of progressively responsible professional student services work experience. One year in the program area to which assigned may be preferred but is not required.
A master’s degree in Counseling, Clinical Psychology, Social Work, or a directly related field may be substituted for one year of experience. A doctorate degree and the appropriate internship or clinical training in counseling or guidance may be substituted for the three years of experience for positions with a major responsibility for professional career or personal counseling.
Education: Equivalent to graduation from a four-year college or university in a related field such as Social Work or Psychology, including or supplemented by upper division or graduate course work in counseling techniques, interviewing, and conflict resolution where such are job-related.
Preferred Qualifications: At least of six (6) months of professional or non-professional experience in a university academic advisement or mentoring at the professional level with students of concern/at-risk college students. At least six (6) months of professional or non-professional experience with and advanced knowledge of crisis intervention, advocacy, and case management. Thorough knowledge of CSU policies and procedures. At least six (6) months of professional or non-professional experience working in a college setting with students who are from a diverse range of cultural and economic background. Additional consideration is given to applicants who have overcome barriers like those confronting program participants as well as other at-risk student populations.
