    Jefferson County Human Services is seeking Long Term Care Case Managers responsible for the client-facing work of this division. Depending on the focus of their team, Long Term Care Case Managers provide a variety comprehensive case management which involves providing individualized support and coordination of services for clients, often with complex needs. This includes conducting assessments to determine eligibility, creating tailored care plans, linking clients to appropriate resources, and maintaining ongoing communication to monitor progress and adjust plans as needed. It requires a thorough understanding of community resources, policies, and regulations, as well as the ability to advocate effectively for clients.

    Experience in a high-volume customer service position demonstrates the ability to manage multiple tasks simultaneously while maintaining a professional and empathetic demeanor. This includes resolving client concerns promptly, addressing inquiries accurately, and handling challenging situations with diplomacy and patience. It highlights skills in communication, problem-solving, and the ability to thrive in a fast-paced, client-focused environment.

    We are hiring Long Term Care Case mangers for the following program:

    IDD (Intellectual and Developmental Disabilities) Program - The teams in this program are responsible for the long-term case management for clients with Intellectual and Developmental Disabilities and their families.

    Schedule :

    The county operates primarily on a 4 day/10-hour work week. (Monday-Thursday ); Human Services works a mix of 4 day/10-hour or 5 days a week (Monday-Friday). This position is hybrid, with remote work and in-office work as determined by team and agency needs.

    Compensation:

    Hiring Range:$ 56,869 - $6 0,000 USD Annually

    Compensation will be determined based on education, experience, and skills.

    Bilingual proficiency in Spanish is an incredible asset to this team and will be compensated upon completion of communication skill test.

    RESPONSIBILITIES

    Long Term Care Case Managers have the following responsibilities:

    • Determine client eligibility by gathering collateral information from hospital, nursing home, long-term skilled home health and home-based services.


    • Complete a face-to-face assessment of clients.


    • Monitoring services to ensure they are meeting the needs of the member


    • Attend court appeal hearings to defend denied applications.


    • Prepare all legal documents .


    • Work with various Medicaid programs (e.g., Home and Community Based Services (HCBS), Elderly, Blind, and Disabled, Mentally Ill, Persons Living with AIDS, Brain Injury, Children’s HCBS, Total Long-Term Care, Program of All Inclusive Care of the Elderly, Home Care Allowance, and Nursing Home programs).


    • Maintain client care plans by reassessing as necessary for most cost-efficient service plan.


    • Adhere to state cost containment regulations.


    • Ensure family members are fully informed of options available.


    • Make referrals as necessary.


    • Consult with state agents.


    • Negotiate rates and plans with Medicaid providers and other providers.


    • Complete billing paperwork.


    • Other duties and responsibilities as assigned.


    QUALIFICATIONS

    Minimum qualifications:

    • Bachelor’s Degree in a related field


    • One (1) year of work-related experience in comprehensive case management and a high-volume customer service position


    • Or an equivalent combination of education and experience.


    Preferred Qualifications:

    • Bilingual in Spanish highly preferred


    • Knowledge of laws relating to children and families


    Knowledge, Skills and Abilities that will lead to success in this role:

    • Excellent individual, group and family counseling and crisis management skills.


    • Able to learn more than one complex computer system and is patient in the learning process.


    • Ability to problem solve by using critical thinking and utilizing resources/researching to find solutions that are best for the family and the department.


    • High level organizational and time management skills and thrives with strict deadlines.


    • Strong customer service and interpersonal skills with the ability to adapt communication based on level of program knowledge of the audience.
