The JobArrowhead Regional Medical Center (ARMC) is looking for a Contract Behavioral Health Care Managerto aide and support in the hospital's Enhanced Care Management (ECM) program. This position will provide support to assigned ECM Teams and engage in outreach activities to enroll patients into ECM supportive programs using various strategies.
Duties include, but are not limited to, the following:
- Maintain contact with ECM team and CAs to remain informed on current team active patient caseloads.
- Assist in initiating initial CHA, PHQ9, BAM, and other assessments as appropriate.
- Maintain contact with ECM team CA to determine team's need for assistance in contacting disengaged patients to meet program metrics and ensure patient support.
Enhanced Care Management (ECM) The
Enhanced Care Management (ECM) benefit is a foundational component of CalAIM. ECM is a whole-person, interdisciplinary approach to comprehensive care management that addresses the clinical and non-clinical needs of high-cost, high-need managed care members through systematic coordination of services that is community-based, interdisciplinary, high-touch, and person-centered.

CONDITIONS OF EMPLOYMENT Appointment: This is a
contract position which
does not obtain status as a regular County employee. Working as a contract employee does not preclude you from applying and competing for Regular positions within the County (Refer to the County of San Bernardino website for current open recruitments).
Work schedule: The work schedule is approximately 40 hours per week.
Travel: Travel throughout the County may be required. At the time of hire, reliable transportation, a valid California Class C driver license and proof of automobile liability insurance is required for the individual providing the transportation. Mileage reimbursement is available.

Minimum RequirementsApplicants must qualify under one of the following options:Option A: A Bachelor's degree in Social Work (BSW), which included a supervised social work internship, practicum, or field work experience.*
Option B: A Bachelor's degree in behavioral science, psychology, sociology, social services, human services, behavioral anthropology, gerontology, cultural and gender studies, or human development*
AND 480 hours of experience obtained in a supervised internship, practicum, or equivalent work or field work experience providing mental health, substance use disorder, or social work services such as interviewing and referring clients, non-clinical counseling, and social casework services in a human services agency. (
Note : Candidates with degree titles not specified above (e.g. Criminal Justice, Political Science, etc.) may be considered under Option C below and must provide coursework for review.)
Option C: Thirty (30) semester (45 quarter) units of completed college coursework as described in Option B,
half of which must be upper division AND one (1) year of experience equivalent to an entry-level social worker. Equivalent duties include primary responsibility for interviewing and referring clients, non-clinical counseling, and providing social casework services as determined by Human Resources.
* Candidates in Final Year of Bachelor's Program : Applications will be accepted from individuals who are within 90 days of completing a qualifying degree (Option A or B), provided they meet minimum requirements for experience at the time of application. Job offers may be made contingent upon degree completion; verification of qualifications will be required prior to hire.
Required Documentation: A list of completed coursework or copies of transcripts
must be submitted with the application unless the applicant possesses a Bachelor's degree in one of the specified areas. Qualifying degrees or coursework must be completed at a college or university accredited by an accrediting association recognized by the United States Department of Education. BSW/MSW degrees or coursework must be approved by the Council on Social Work Education.
Degrees or coursework completed outside of the U.S. must be accompanied by an evaluation report from a reputable credential evaluation service indicating equivalency to degrees or coursework completed at a college or university accredited by an accrediting association recognized by the United States Department of Education. Proof of the required completed coursework and/or degree(s) will be required at the time of the interview.
Desired QualificationsThe ideal candidate will have at least two (2) years of experience working in an acute care hospital as a Social Worker. This person will be accustomed to working in a diverse environment and enjoy working with people of the community.
