    Description

    Provides care to medically complex patients in collaboration with an interdisciplinary team. Works autonomously and has authority, responsibility, and accountability for assessment, planning, implementation and evaluation of wound, ostomy, and continence care program throughout the inpatient care areas. Serves as a resource for wound care management; including identification, assessment, treatment, and prevention. Provides specialized consultation on wound, ostomy, and continence issues for patients, families, nursing, and medical staff. Serves as a consultant in the treatment and management of patients with draining wounds, fistula and tubes, and patients with altered skin integrity due to pressure necrosis, drainage, or incontinence. Works with interdisciplinary staff to provide updated information regarding wound care. Conducts rounds on patients with wound/ pressure ulcers on a daily and as needed basis. Co-chairs wound prevention meetings and works on initiatives to support nurse-driven protocols on wound care.

    BILINGUAL INCENTIVE: Some positions may be eligible for bilingual incentive depending on the applicable memorandum of agreement and the needs of the department. To qualify for this incentive, incumbents in eligible positions must take and pass the applicable bilingual fluency examination.

    EDUCATIONAL INCENTIVE: Some positions may be eligible for educational incentive pay in addition to the adjusted base rate of pay in the noted amounts for educational attainments which are not specifically required for the classification:
    • Bachelor's Degree $1.25/hr
    • Graduate's Degree $1.75/hr
    PAYROLL TITLE: Per Diem Registered Nurse III

    AGENCY/DEPARTMENT: Health Care Agency - Ventura County Medical Center

    Per Diem Registered Nurse III are represented by the California Nurses' Association (CNA) and are eligible for overtime compensation.

    NOTE: Salary placement will be determined according to the current CNA memorandum of agreement.

    The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies for this position only. There is currently one (1) Regular vacancy at Ventura County Medical Center.

    Examples Of Duties

    Duties include but are not limited to the following:
    • Implements nursing processes (assessment, planning, implementation of care plan, evaluation of patient response); revises care plans as needed
    • Administers basic and advanced competency nursing care in accordance with VCMC/SPH policies and procedures and Patient Bill of Rights
    • Provides direction to others regarding complex health and patient problems
    • Provides patient and staff education including the coordination of educational programs
    • Evaluates progress toward attaining outcomes for the patient or the patient’s condition
    • Participates in hospital and/or nursing administration care committees, acting as chairperson if appropriate
    • Reviews, identifies, reports, and corrects deficiencies in practices, products, or equipment, as necessary
    • Develops, implements, and documents performance improvement processes
    • Recognizes problem situations and takes appropriate steps leading to correction
    • Serves as a preceptor for nurses cross training or training in specialty
    • Collects and analyzes relevant data pertinent to the patient’s health or situation used in determining care issues;
    • Develops and implements a plan that prescribes interventions to attain outcomes; coordinates care delivery, and employs strategies to promote health and a safe environment
    • Collaborates with the team of patient, family, and healthcare providers in providing patient care in a safe, healing, and caring environment
    • Provides learning opportunities for patients/ family members and team members
    • Directly provides health information to patients, families, and treatment team
    • Maintains appropriate supply of equipment and materials
    • Maintains documentation of patient status, intervention, and responses
    • Provides resources to educate staff on wound care and assessment, and management
    • Maintains and improves knowledge level related to wound care
    • Evaluates wound care devices introduced to the hospital and assists in training staff
    • Performs yearly Performance Improvement projects related to wound care
    • Performs other related duties as required.


    Typical Qualifications

    These are entrance requirements to the examination process and assure neither continuance in the process nor placement on an eligible list.

    EDUCATION, TRAINING, and EXPERIENCE
    Requires five (5) years of full-time professional registered nursing experience including two (2) years of full-time experience in Wound and Ostomy Nursing. Otherwise, 5 years of full-time professional nursing experience demonstrating current competencies and maintaining current certifications necessary to work in the assigned units/clusters.

    NECESSARY SPECIAL REQUIREMENTS
    • Must possess and maintain a current, valid license as a Registered Nurse issued by the State of California
    • Must possess a current, valid Basic Life Support (BLS/CPR) certification
    • Intermediate experience using Microsoft Office products such as, Excel and Outlook
    • Intermediate experience using Cerner
    DESIRED
    • Wound Care Certification
    • Completion of an accredited Wound, Ostomy, Continence Nursing (WOCN) program preferred upon hire.
    • Bachelor of Science in Nursing
    • Master of Science in Nursing
    Knowledge, Skills, and Abilities:
    Comprehensive knowledge of: the principles, practices, and techniques used in advanced nursing care of wound care and ostomy; Federal, state and local laws, rules, regulations, policies, processes, and mandated procedures pertinent to wound care and ostomy.

    Working knowledge of: Microsoft Office products such as, Excel and Outlook; Cerner EHR.

    Working ability to: provide direction to others; self-direct continuing education needs, provide a positive role model and public image; work proficiently in emergency and/or stressful situations; maintain confidentiality of patient/client information; effectively establish and maintain working relationships with medical staff, co-workers, patients and the public; and communicate effectively, both orally and in writing.
