DESCRIPTIONThe City of Long Beach, Department of Health and Human Services is recruiting for a full-time, unclassified, at-will Public Health Professional I(PHP I) to serve as the Re-Entry Specialist in the Homeless Services Bureau . Reporting to the Jail Clinician (PHP III), this position will provide mental health services and reentry service navigation to people experiencing homelessness throughout the City of Long Beach.
EXAMPLES OF DUTIES Reentry and Case Management Duties: - Accept referrals from the Jail Clinician to provide wraparound services and case management to high-utilizers of the justice system, with the goal of reducing recidivism and promoting long-term stability.
- Collaborate on jail discharge planning and placement, ensuring individuals are connected to appropriate housing, services, and support systems upon release to prevent homelessness and reduce recidivism.’
- Develop and lead innovative counseling programs, such as groups, workshops, and skill-building sessions, to support reentry efforts and community reintegration.
- Assist care team with housing resources to persons with severe physical, mental health, or substance use needs.
- Participate in case conferencing and care coordination for higher-barrier clients to assist in navigating local and county homeless and healthcare systems.
- Navigate the Long Beach Coordinated Entry System to link clients to housing resources, including utilizing the Homeless Management Information System (HMIS) and other data platforms to document all client interactions in a timely manner.
- Foster and develop collaborative working relationships with community and city reentry services, community-based organizations specializing in mental health, and other social service agencies.
- Provide case management to the reentry population, including consultation with probation/parole departments and submitting treatment summary reports for court advocacy.
- Assist in the development of a reentry action plan to establish the Multi-Service Center (MSC) as an access site to effectively serve formerly incarcerated persons and their families.
- Serve as a liaison—along with the Reentry Coordinator—to county and state reentry efforts to increase resources for community-based organizations serving the reentry population in Long Beach.
- Assist with the implementation of community restorative justice practices and trauma-informed approaches to reduce family violence and strengthen community pathways for family reunification.
- Assist with the development of grant proposals, contract management, and reporting, along with the Reentry Coordinator, for dually impacted individuals experiencing homelessness and reentry.
- Other duties as assigned.
REQUIREMENTS TO FILE EDUCATION - Bachelor's degree in Public Health, Community Health, Public Administration, Business, or closely related field. (Proof of education required)
- Experience offering specific and substantial preparation for the duties of the position may be substituted for the required education on a year-for-year basis.
EXPERIENCE - One-year of full-time professional experience in public health or human services related to a specialized program.
- A valid driver’s license, or the ability to arrange necessary and timely transportation for field travel.
DESIRABLE QUALIFICATIONS - Professional experience in reentry services, homeless services, case management, or a related field.
- Knowledge of the criminal justice system, reentry processes, and community resources for incarcerated/formally incarcerated individuals.
- Experience with case management systems, such as the Homeless Management Information System (HMIS), or similar databases.
- Strong communication and collaboration skills with the ability to work effectively with community-based organizations, government agencies, and multidisciplinary teams.
- Familiarity with trauma-informed care, restorative justice practices, and wraparound service models.
- Ability to work independently and manage multiple priorities in a fast-paced environment.
- Bilingual skills (preferred), particularly in Spanish or another language commonly spoken in the community.
