The purpose of positions in this class is to provide outreach and care management services to individuals in need and their families by assessing participant needs, providing information and referrals on resources, determining participants' qualification for various available programs, and developing, coordinating and monitoring care plans.
The work typically is performed under the supervision of clinically licensed staff in higher classifications.
This position is Extended Part-time with a schedule requirement of up to 32 hours per week.
Minimum Training and Experience Required to Perform Essential Job Functions - Bachelor's degree in social work, gerontology, human services, social ecology, marriage and family counseling or closely related field and two (2) years of experience in social work outreach or counseling or any combination of education and experience that provides equivalent knowledge, skills and abilities.
- Possession of a valid California driver license.
- Possession of, or ability to obtain, First Aid/CPR/AED certification within three months of employment.
Desired Qualifications- License in Clinical Social Work or Marriage/Family Therapy
- Graduate level experience and education

- Able to analyze complex social and economic issues and apply problem-solving strategies.
- Skilled in synthesizing assessments to make appropriate referrals and service matches.
- Exercises sound judgment and remain calm and decisive in emergencies.
