    Description

    Under general direction and following established medically-approved policies and procedures, the incumbent implements specialized skilled nursing processes in designated hospital units of Ventura County Medical Center and Santa Paula Hospital.

    The Senior Registered Nurse-Hospital works collaboratively with a medical doctor to coordinate and screen for the appropriateness of admissions and continued stays. They make recommendations to the physicians for alternate levels of care when the patient does not meet the medical necessity for inpatient hospitalization. They interact with the family, patients, and other disciplines to coordinate a safe and acceptable discharge plan. They function as an indirect caregiver, patient advocate, and manage patients in the most cost-effective way without compromising quality. They will also be responsible for complying with AB 1203, Post Stabilization notification.

    PAYROLL TITLE: Senior Registered Nurse-Hospital

    WHAT WE OFFER:

    The County of Ventura offers an attractive compensation and benefits package, subject to eligibility requirements, that includes:
    • Educational/Bilingual Incentive: Possible educational incentive of 2.5%, 3.5%, or 5% based on completion of Associate's, Bachelor's, or Master's degree. Incumbents may also be eligible for bilingual incentives depending upon operational need and certification of skill.
    • Certification Pay: $0.813 per hour based on scheduled work week hours for each qualified certification up to a maximum of five (5) certifications. One of the five certifications may be paid at $2.00 per hour for a National Certification. List provided in the MOA, Exhibit 9
    • Evening Shift Differential - 7.5%
    • Night Shift Differential - 15%
    • Weekend Shift Differential - $2.00 per hour
    • Continuous County Service Incentive Pay: 1% for 7 years, 1.25% for 12 years and 1.5% for 17 years
    • Preceptor Pay - $2.50 per hour
    • House Supervisor Pay - $4.00 per hour
    • Charge Nurse Premium Pay - $1.50 per hour
    • Float Nurse Premium Pay - $3.50 per hour
    • In-House Registry Pay:
    • Non-overtime is 1.75 straight time base hourly rate
    • Overtime is 2 times base pay straight time base hourly rate
    • Standby Premium Pay - ¼ of regular hourly wage or Federal minimum wage (whichever is greater)
    • Flexible Credit Allowance: Biweekly contribution towards medical, dental, and/or vision insurance from authorized plans.
    • Pension Plan: Participation in the County's defined benefit plan.

    Senior Registered Nurse-Hospital is represented by the California Nurses' Association (CNA) and is eligible for overtime compensation. Salary placement will be determined according to the current CNA memorandum of agreement.

    The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term), Intermittent, and Extra Help vacancies. There is currently one (1) Regular vacancy.

    Examples Of Duties

    Duties include, but are not limited to the following:

    • Plans, develops, assesses & evaluates care provided to members;
    • Collaborates with physicians, other members of the multidisciplinary health care team & patient/family in the development, implementation & documentation of appropriate, individualized plans of care to ensure continuity, quality & appropriate resource use;
    • Recommends alternative levels of care & ensures compliance with federal, state & local requirements;
    • Assesses high-risk patients in need of post-hospital care planning;
    • Develops & coordinates the implementation of a discharge plan to meet patients' identified needs;
    • Communicates the plan to physicians, patient, family/caregivers, staff & appropriate community agencies;
    • Reviews, monitors, evaluates & coordinates the patient's hospital stay to assure that all appropriate & essential services are delivered timely & efficiently;
    • Participates in the bed huddles & carries out recommendations congruent with the patient's needs;
    • Coordinates the interdisciplinary approach to providing continuity of care, including Utilization management, Transfer coordination, discharge planning, & obtaining all authorizations/approvals as needed for outside services for patients/families;
    • Conducts daily clinical reviews for utilization/quality management activities based on guidelines/standards for patients in a variety of settings, including outpatient, emergency room, & inpatient facilities;
    • Acts as a liaison between in-patient facility & referral facilities/agencies & provides case management to patients referred;
    • Refers patients to community resources to meet post-hospital needs;
    • Coordinates transfer of patients to appropriate facilities; maintains & provides required documentation;
    • Adheres to internal & external regulatory & accreditation requirements & compliance guidelines, including but not limited to: The Joint Commission, Department of Health Services, Centers for Medicare & Medicaid Services, and Department of Managed Health Care;
    • Educates members of the healthcare team concerning their roles & responsibilities in the discharge planning process & appropriate use of resources;
    • Provides patients with education to assist with their discharge & help them cope with psychological problems related to acute & chronic illness;
    • Per established protocols, reports any incidence of unusual occurrences related to quality, risk and/or patient safety which are identified during case review or other activities;
    • Reviews, analyses & identifies utilization patterns & trends, problems or inappropriate utilization of resources & participates in the collection & analysis of data for special studies, projects, planning, or for routine utilization monitoring activities;
    • Coordinates, participates & or facilitates care planning rounds & patient family conferences as needed;
    • Participates in committees, teams or other work projects/duties as assigned; and
    • Performs other related duties as required.


    Typical Qualifications

    These are entrance requirements to the examination process and assure neither continuance in the process nor placement on an eligible list.

    EDUCATION, TRAINING, and EXPERIENCE
    Requires two (2) years of full-time professional registered nurse experience, including two (2) years in an acute care setting.

    NECESSARY SPECIAL REQUIREMENTS
    • Must possess and maintain a current, valid license as a Registered Nurse issued by the State of California.
    • Must have a current, valid Basic Life Support (BLS/CPR) certification by first day of employment.
    • Must be able to work collaboratively with the Multidisciplinary team, multitask and work in a fast-paced environment.
    Knowledge Skills and Abilities:

    Comprehensive knowledge of the following:
    • of regulatory requirements and accreditation standards. (TJC, Medicare, Medi-Cal, etc.)
