    DESCRIPTION OF DUTIES/ESSENTIAL FUNCTIONS
    The Houston Health Department is actively searching for a skilled individual to perform the following essential functions as a Public Health Nurse IV.

    • Collaborates with local hospitals and private providers to deliver education on Tuberculosis (TB) diagnosis and treatment in accordance with Texas Department of State Health Services (DSHS) Standards of Disease Management (SDO). Maintains clear, accurate, and timely documentation of all encounters and consultations with approved TB experts and TB providers.
    • Under general supervision, provides telephonic case management as required by the Bureau of Tuberculosis Control. Delivers comprehensive, holistic nursing care across a variety of settings, including health centers, patients’ homes, and correctional facilities.
    • Coordinates and manages nursing care by developing, implementing, and maintaining complete and accurate case management plans for assigned patients. Assists patients in carrying out care activities in accordance with established case management plans.
    • Collects bacteriological specimens and blood samples as needed; monitors laboratory results; and ensures appropriate follow-up, treatment coordination, and patient education. Provides TB-related education to patients, internal staff, and external partners.
    • Administers, reads, and interprets Tuberculosis screening tests, including Mantoux and Interferon Gamma Release Assays (IGRA), and conducts clinical assessments to determine appropriate follow-up and referrals.
    • Triages patient medications and monitors for adverse reactions or side effects. Applies nursing judgment and the nursing process to assess, intervene, and refer patients in accordance with established policies and procedures. Makes appropriate referrals for additional medical, behavioral health, or social services as indicated.
    • Travels between work sites using a personal or City-issued vehicle as required. Must possess and maintain a valid driver’s license in compliance with City of Houston Administrative Policy (A.P.) 2-2.
    WORKING CONDITIONS

    The position occasionally requires stooping or bending. Occasional very light lifting, such as three or four reams of papers or books (up to 20 pounds or an equivalent weight) may be required. There are occasional minor discomforts from exposure to less-than-optimal temperature and air conditions. The position may involve dealing with modestly unpleasant situations, as with occasional exposure to office chemicals and/or extensive use of a video display terminal.

    MINIMUM REQUIREMENTS

    EDUCATIONAL REQUIREMENTS
    Requires graduation from an accredited school of nursing. A Bachelor of Science degree in nursing is preferred.

    EXPERIENCE REQUIREMENTS
    Three (3) years of registered nursing experience required.

    SUBSTITUTION
    A Master of Science degree in Nursing may substitute for two (2) years of the experience requirement.

    LICENSE REQUIREMENTS
    Must be licensed to practice as a registered nurse (RN) in the State of Texas. CPR certification is required. RN License and CPR certification must be maintained and current for continued employment.

    PREFERENCES

    **Preference shall be given to eligible veteran applicants provided such persons possess the qualifications necessary for competent discharge of the duties involved in the position applied for, such persons are among the most qualified candidates for the position, and all other factors in accordance with Executive Order 1-6.**

    Grants & Special funded positions are dependent upon continued available funds. If funding is no longer available, the employee may be laid off or transferred. Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in this classification.***
